This application is for renewal projects only. One project application must be completed for each renewing project submitted for funding consideration. Please respond to all questions in this section. Please note that several scored factors are calculated according to past performance data drawn from recent Annual Performance Reports (APRs) and other grant management data. No additional narratives about past performance or grants management are required. The Collaborative Applicant will verify your organization's CoC participation, which is also a scoring factor.
