Application Summary

To get started, log in to your ZoomGrants account, click "Apply Now" and complete the information on the Application Summary tab. After completing this tab, navigate to the Pre-Application tab to complete the pre-application form and upload pre-application documents. The pre-application must be submitted for review before January 23, 2026. We recommend early submission of the pre-application to provide ample time to work on the full application. Question guidance

• Project Title - Enter a general project description that defines the overall intent of the project. Example: "Cardiac Monitor Replacement Project". 
• Amount Requested - Enter the dollar amount requested from the state. The state share is 50% of the total cost of your grant project unless your agency is applying for a financial waiver to the standard 50% match or applying for system improvement funding. Line items will be entered later on the Detailed Budget tab after your pre-application is approved. Please return to this tab to update the final amount requested from your detailed project budget.
• Applicant Match Amount - Enter the dollar amount your agency will be responsible for. Your agency's match is 50% of the project total unless you are applying for a financial waiver to the standard 50% match.
• Applicant Information - Enter your name and contact information as the primary grant contact. 
  
  • Primary grant contact - The person responsible for completing and submitting this application in ZoomGrants. If your project is funded, you must use the same account that you applied with. 
• Organization Information - Enter your organization's legal name as found on your W-9 form. Click Add Address to verify and save your organization's address.
• Authorized Signer - Enter the name and contact information of a person with signature authority for your organization. This person must have the authority to execute legally binding agreements for your organization and will sign the provider grant attestation attachment for this application. Common examples of authorized signers include but are not limited to the Board President, City Manager, County Manager and CEO.
• Collaborators - Can be assigned below. Collaborators can edit the application but can't submit the application using their ZoomGrants account.

Grant workshops

Informational grant workshops are scheduled from December through January 2026. Grant workshops aim to educate potential applicants and answer questions about the program. Visit: https://cdphe.colorado.gov/emergency-care/funding/emergency-medical-and-trauma-services-grant-workshop-schedule to find a list of workshops. Various RETACs host grant workshops in different regions of the state. All potential grant applicants are welcome to attend any RETAC workshop session. 

Tips for a successful application

• Review the EMTS Grant Funding Guide before starting.
• ZoomGrants will time out after a period of inactivity. Save your work often by clicking outside of the question you are working on.
• Gather the financial statements and required documents listed on the Pre-application tab.
• Use your browser's zoom function if you cannot see the full application page.
• Use your browser's refresh function to update calculations in the detailed project budget tab and ensure your progress has been saved.
• The Organization Information section below must reflect the information for the primary organization that will be responsible for administering the grant project/funds.
• Return to the Application Summary tab to update the STATE amount requested after you've completed the Detailed Project Budget tab.
• We recommend submitting the full application at least a week before the final deadline.

Submission deadlines 

Applications submitted after these deadlines will be rejected.

Pre-application: January 23, 2026

Full Application: January 31, 2026

Before submitting the pre-application and full application, ensure that all questions have complete responses, budgets are entered correctly and attachments are in PDF format. If you need to make corrections after submission, email Andre Smith at andrek.smith@state.co.us to request that the application be reopened.

Application Questions

To get started, select the funding type and funding category(ies) included in your project. Your selection in question #1 will determine which questions are shown throughout the application. Refer to the EMTS Funding Guide or contact us if you need help selecting the correct funding type or category. Only choose the grant categories pertinent to your project.

Each answer you provide below must address each grant category included in your proposed project. Reviewers are instructed to score your application based on the guidelines in either the provider grant scoring tool or the system improvement funding scoring tool. Failure to provide a unique and complete answer to all pertinent questions will cause your application to receive a lower score. 

Upload documents that support your project by clicking 'Upload' in the slots below.  Vendor quotes, cost estimates, letters of support or other supporting documents can be uploaded on this page. Name your files with the following prefixes:

• QUO_ Quotes and cost estimates
• LOS_ Letters of support
• OTHER_Other supporting documents

All uploaded documents must be in PDF format and unlocked, or they will be considered missing. At least one quote or cost estimate is required. The quotes you include should be concurrent with the items entered on the Detailed Project Budget tab.

For each file upload, a new window will pop up. You must enter a file description and select a file to upload from your computer before clicking Upload Now. The file size limit is 4 MB. Use the Refresh Page button to update the list of files uploaded.

The Supporting Documents Upload tab is the final step in the application. Before clicking the "Submit Now" button, check the following:

• Ensure that all application questions have complete responses.
• Ensure your budget tables accurately list the quoted item costs.
• Ensure your budget tables accurately list the number of items requested.
• Ensure your budget tables include all items required to complete your project.
• Ensure all uploaded documents are in PDF format and are unlocked.
• Please return to the Application Summary tab to input a finalized amount requested.
  
  • This will help reviewers and staff accurately identify the total amount requested.

When ready to submit the application, click the green Submit Now button on this page. If you need to make corrections after submission, you must email Andre Smith at andrek.smith@state.co.us to request the application be reopened.

Submit your quarterly progress report on this tab. Provide responses to the questions below to let us know the status of your grant-funded project.

Progress reports must be submitted quarterly.

Report Due Dates

• Sept. 30
• Dec. 31
• March 31
• June 30

Progress reports must be submitted before your final reimbursement request can be approved. Please do not upload reimbursement requests to this report. Reimbursement requests and the required documentation must be emailed to the address below.
