The ESG Program is administered by the Department of Housing and Urban Development (HUD) and is authorized by the McKinney-Vento Act and amended by the HEARTH Act. The purpose of the ESG Program is to provide funding to: Engage homeless individuals and families living on the street, improve the number and quality of emergency shelters for homeless individuals and families, help operate shelters, rapidly re-house homeless individuals and families, and prevent families and individuals from becoming homeless. ESG funds administered by Clark County are being made available to eligible nonprofit organizations that provide critical services to individuals and families that are homeless or at risk of homelessness in Southern Nevada. This grant cycle there will be an emphasis on funding projects that serve these IDENTIFIED NEEDS: shelter facilities and rapid rehousing programs serving victims of domestic violence, youth and transition age youth, and INTACT families. Organizations awarded funds through this application are required to receive individuals and families through the coordinated intake process and utilize HMIS for data entry.

Clark County is interested in supporting specialized programs to fill gaps in services for underserved populations. The funded activities for this funding cycle are Emergency Shelter and Rapid ReHousing. Applications for any other actvities will be declined.

3. Choose the homeless/ low-income special population that best describes the clients served by your program. If you are emphasizing resources for a specific group please explain in the project description. Some of these groups overlap, you may check off more than one but the project description (next question) should explain how you will serve that population(s)
4. Describe the program for which you are applying for funds.
Include the following information: service activity plan, intake and referral procedures, program procedures and evaluation plan, intended accomplishments. Include the target population, services, housing, partnerships, and how the program is appropriate for the populations you serve, etc. If you will be providing emergency shelter, please indicate if it is a low barrier to entry program.
5. Select the Continuum of Care and federally identified needs that your project addresses. Your project description and outcomes must support this information. Housing/services provided must be appropriate for type of population.
6. Briefly describe the need, local situation, or conditions that your project will address. Describe how your project will meet the priority needs of the local CoC. Include local data from the homeless census, HMIS, needs assessments, and other relevant data.
7. Describe the outcomes and accomplishments your project(s) has achieved. If there were difficulties, describe how that will be overcome in the future. Indicate how long your organization has implemented this program. If project is new, provide the outcomes and accomplishments for a similar project/program(s) your agency administers.
8. Describe how your agency will carry out case management. Will it be provided by your agency staff or through collaboration with another provider? Will a portion be paid through this grant? Other sources? Are funds committed? Under the Interim Rule, case management is required for projects funded for homeless prevention and rapid re-housing activities.
9. Describe opportunities that clients have to provide input in program evaluation of services and/or everyday delivery of services.

14. Engagement with regional efforts on homelessness: Indicate approximately how many times in the last year your agency has attended the following meetings in the last 12 months.
15. Explain your agency's role in PHC, Homeless Census (Point In Time count), Family Connect, centralized & coordinated entry planning, and any other regional collaborative efforts.
16. How does your agency plan to ensure project sustainablity? How will you ensure that you meet the match requirement? Dollar for dollar match of ESG funds is required. It is also highly recommended that your project have at least 3 months of cash reserves.
