Columbia Pacific CCO Approach

At Columbia Pacific Coordinated Care Organization, our mission is to inspire local connections that foster member health and community resilience. With a primary focus on Oregon Health Plan members, Columbia Pacific CCO invests in clinical and community capacity building for prevention, service integration, affordable housing, community-identified health priorities and health equity as core components of health care transformation. We recognize the importance of meeting immediate community needs while also working for changes in policies, systems and institutional practices that are barriers to transformative health practices.  We recognize that this kind of change requires more than a single year’s effort and a single organization’s work.

Therefore, we seek proposals that leverage community resources, expertise, and other funding opportunities through partnerships among organizations with aligned goals and efforts. We encourage collaborations that implement best practices and build toward sustainable programs with measurable results.

All proposals should provide evidence of partnership between the Applicant and community or other organizations. Proposals should also clearly advance the priorities and goals of the Regional Health Improvement Plan, Trauma Informed Care, Health Equity, or CCO Quality Metrics.

As a foundation for Columbia Pacific’s investments, we seek proposals that address health disparities and center on OHP members who are systemically marginalized. We prioritize applications that address and support health equity and trauma-informed approaches.

Columbia Pacific will seek investments in organizations that:

  • Are IRS-designated 501(c)3
  • Are known contributors to local community health and well-being
  • Have effective leadership that engages a base of community support 
  • Are committed to addressing equity and health disparities
  • Demonstrate the administrative capacity, financial plan and multi-organizational commitment to achieve stated goals 
  • Integrate formal evaluation of their work and programs
  • Value systemic and sustainable approaches
  • Include impacted individuals in defining the problem and proposed solutions
  • Include data and evidence, where available, and demonstrate planning
  • Leverage additional financial and in-kind resources

To be considered for funding:

Please address all the following that apply.  Priority will be given to proposals that address a greater number of the criteria below.

  • Financial Sustainability of the project upon cessation of CCO funding
  • Specific Return(s) on Investment Measures of success or outcomes to be achieved
  • Systemic or population impact
  •  Impact on health equity and/or reduction in health disparities
  • Evidence or data supporting the project's likelihood of success
  • Agreement to collaborate with others
  • Community Partnerships
  • Degree of need for the project
  • Evidence of leveraged/braided funding beyond CPCCO’s support

Columbia Pacific will not invest in:

  • Projects that are external to CPCCO’s service area
  • Capital projects, other than on an exception basis as determined by the CCO Board of Directors
  • Projects that represent more than 25% of an organization’s annual budget
  • Projects that have no evidence of or plan for financial sustainability once the CCO’s investment ceases
  • Individuals unauthorized to act on behalf of a qualified tax-exempt organization(s)
  • Individuals operating for their own personal benefit
