WARNING: If you answered "no" to any of the pre-application questions, please do not proceed. The business does not meet the eligibility requirements for this program. 

You must answer all questions. Please answer all questions truthfully and to the best of your ability. At the end of the application, you will be required to declare, under penalty of perjury, that all of the information submitted in the application is true and correct. 

Submit documentation for all options that apply.  Businesses may receive funding under mutliple options.

Option 1

If the business was closed or directly impacted by a public health order as a result of the COVID-19 public health emergency (e.g. see the Weber CARES Program Information document in the Application Resources section above), please submit the following:

• Most recently filed business tax return (2018 or 2019) or for Sole Proprietor: 2018 or 2019 Federal Tax Schedule C
• Form 941 for 1st Qtr. 2020 (if no employees, this is not required)
• IRS Form W-9

Option 2

If the business incurred expenditures due to the COVID-19 public health emergency between March 1, 2020 and May 31, 2020, for items such as changes to buildings to encourage social distancing, purchases of personal protective equipment (PPE), cleaning and sanitation supplies, or other COVID-19 related expenses, please submit the following:

• Most recently filed business tax return (2018 or 2019) or for Sole Proprietor: 2018 or 2019 Federal Tax Schedule C
• Form 941 for 1st Qtr. 2020 (if no employees, this is not required)
• IRS Form W-9
• Receipts, invoices, or other documentation supporting the amounts spent on COVID-19 related items.

Option 3

If your business was significantly impacted by the COVID-19 pandemic, such as decreased customer demand, supplier interruptions, voluntary closures, etc. please submit the following:

• Most recently filed business tax return (2018 or 2019) or for Sole Proprietor: 2018 or 2019 Federal Tax Schedule C
• Form 941 for 1st Qtr. 2020 (if no employees, this is not required)
• IRS Form W-9
• Monthly profit and loss for March, April, and May 2019
• Monthly profit and loss for March, April, and May 2020
• Any other documents to support the effect of the COVID-19 pandemic on the business's net income.
