Welcome to the EMTS Funding Program grant application!

This application opens on December 16, 2020. The required pre-application must be submitted for review before February 8, 2021. Submission and approval of the pre-application is required to continue to the full funding application. Use the tabs above to navigate to the following sections.

Application Review

Grant applications will be scored competitively using 50-point scales. For your reference, you can find these scoring tools in the library above. Provider grant applications will be reviewed and scored by two different committees. The first application review will be performed by your specific RETAC at a date and time determined by the RETAC. The second review session will be conducted by the SEMTAC on May 13 & 14 2021.  All applicants are encouraged to attend both their RETAC and SEMTAC reviews to answer any questions reviewers may have. Review schedules will be posted at: https://cdphe.colorado.gov/emts-grant-review-schedule after the grant application deadline.

System improvement funding requests will be reviewed and scored by the SEMTAC Public Policy and Finance Committee on April 7, 2021.

Applicants applying for a waiver of the standard 50 percent cash match requirement must go through a financial waiver review on February 24, 2021. Financial waiver requests must be reviewed and approved by the financial waiver review committee. Applications with an approved financial waiver are advanced for consideration at their RETAC and SEMTAC review sessions. Applications that fail waiver review do not advance to the evaluation process.

Tips to get started

• Review the EMTS Grant Funding Guide in the library above before starting.
• Gather the required documents for upload into the pre-application.
• Use your browser's zoom function if you cannot see the full application page.
• ZoomGrants will time out after inactivity. Save your work often by clicking outside of the question you are working on.
• Use your browser's refresh function to make sure your progress has saved and to update calculations.
• The "Project Title" below should reflect the overall intent of your request and not include your organization name.
• The "Organization Information" section below must reflect the information for the primary organization that will be responsible for the administering the grant project/funds.
• The EIN information number you list must match the number listed on the W-9 form that you'll submit on the next tab.
• The "Authorized Signer" you list must be a person who can execute legally binding agreements for your organization. This person also cannot be the primary grant contact and/or person filling out this application. Examples of authorized signers include but not limited to: Board President, CEO, County Manager, City Manager.
• The "IRS Verification" feature only searches non-profit entities. If your EIN is not found, please proceed on through the application.
• We recommend submitting your full application at least 1 week before the final deadline.

Now that your pre-application is approved, you can get started on completing the full funding application. The application questions on this tab must be answered for review by the RETAC and SEMTAC scoring committees. Your answers to each question must address all aspects and categories of the proposed project. Failure to answer all pertinent questions will cause a lower score from grant reviewers. Reviewers will use the scoring tools found in the library below to score your application.

To get started, select the funding category type(s) that you are applying for. Your selection will determine the questions that are shown. 

Upload documents which support your project by clicking 'Upload' in the slots below.  Upload vendor quotes, costs estimates, letters of support or other supporting documents. Name your files with the following prefixes:

• QUO_ Quotes and cost estimates
• LOS_ Letters of support
• OTHER_Other supporting documents

All uploaded documents must be in PDF format and must be unlocked or they will be deemed missing. At least one quote or cost estimate is required. For each file upload, a new window will pop-up. You must enter a file description and select a file to upload from your computer before clicking "Upload Now". The file size limit is 4 MB. Use the 'Refresh Page' button to update the list of files you've uploaded.

Submit the required fiscal year 2022 progress report on this tab. Please provide responses to the questions below to let us know the status of your grant-funded project.

Progress reports must be submitted quarterly until your project is complete.

A final progress report must be submitted before your final reimbursement request is approved.
