Please provide the following information for your organization: As an Applicant in the ZoomGrants system, you will only have to complete this information once. Your information will be saved for all other applications submitted through the ZoomGrants system. Project applicants are required to have an active Unique Entity Identifier (UEI) and an active registration in the Central Contractor Registration (CCR)/System for Award Management (SAM.gov) to apply for funding under the HUD CPD CDBG and HOME programs. Additionally, please provide the Employer Identification Number (EIN), also known as the Federal Employer Identification Number (FEIN) or the Federal Tax Identification Number. Lastly, please provide your organization’s CAGE Code.

What is an UEI? SAM.gov has replaced the Data Universal Numbering System (DUNS) with UEIs as a unique identifier for entities throughout the federal awarding lifecycle, in SAM.gov, in other Integrated Award Environment (IAE) systems, on required forms, and in downstream government systems. The UEI is a 12-character alphanumeric identifier used in SAM.gov and other federal government systems to identify a unique entity. This number is used in reporting under the Federal Funding Accountability and Transparency act (FFATA), which requires the federal Office of Management and Budget (OMB) to maintain a single, searchable website that contains information on all Federal spending awards. As a condition of receiving federal funding, you are required to provide a UEI number, which you will then need to be able to obtain a CAGE Code. If your organization does not yet have a UEI number (or if it is unknown) visit SAM.gov to start the process of requesting a number.

What is an EIN? The Employer Identification Number (EIN), also known as the Federal Employer Identification Number (FEIN) or the Federal Tax Identification Number, is a unique nine-digit number assigned by the Internal Revenue Service (IRS) to business entities operating in the United States for the purposes of identification.

What is a SAM CAGE Code and how do I get one? SAM stands for System for Award Management, which is the primary registrant database for the U.S. Federal Government. SAM collects, validates, stores, and disseminates data in support of agency acquisition missions. Every organization receiving federal funds must be registered in SAM. Your Cage Code identifies and helps verify your current registration in SAM, which must be updated annually. For more SAM Registration information Click Here or go to https://www.sam.gov

Registering for an UEI and CAGE Code on SAM.gov is free of charge. If you encounter any organizations or websites soliciting a fee or charge to acquire a UEI and/or CAGE Code or to charge you for registering in sam.gov it is likely a scam or fraudulent!

Please be sure to answer each question thoroughly. If a question is not applicable, please write "N/A" and a brief explanation, as the system will not allow you to submit your application if any question is left blank. Additional information is described in italics below each question. Additional instructions and resource materials can be found in the Solicitation Guide and may be accessed under the "Library" tab. If you submit your application, you may still make changes to your application up to the deadline. All changes made to your application after submittal, but before the application deadline, will automatically be saved - there is no need to resubmit.

Please provide the following documents with your application for funding:

• For all applications: Provide a one paragraph summary of the project that will be used to present the project to elected officials and the public.
• For all applications: Acknowledgements and Certifications signed
• For all applications: Risk Rating Review
• For all applications: Resolution of Support from the applicable Ramsey County municipality OR for nonprofit organizations upload the board resolution allowing for the application of grant funds. Please provide written statement if your project cannot obtain a board resolution until after application submissions are due. 
• For all applications: Project Management Capacity Documentation Provide the name(s) of Internal personnel or staff persons(s) responsible for carrying out the project, and a description of experience with similar projects and ability to complete the project on time;
• For Public Facilities, Non-Residential Projects: Maps showing the project location, nearby facilities and distances, if applicable. Project Maps should indicate the Project Site AND/OR the Program Service Area. Please also indicate the Census Tract and Block Groups within which the project and its service area are located.
• For Public Facilities, Non-Residential Projects: Preliminary drawings and site plan, where applicable. If the rehabilitation work will change the building foot print in anyway, then this attachment is required.  
• For Public Facilities, Non-Residential Projects: Photos of the project site and any existing buildings. 
• For Public Facilities, Non-Residential Projects: Public Facilities Compliance Acknowledgement. 
• Documentation of Site Control
• For Public Facilities Projects: Provide any Environmental Review documentation and mitigation plans.
• For Public Facilities Projects: Upload NEPA Checklist. 
• For Public Facilities Projects: Financial Proformas Provide in your own format.
• For Public Facilities Projects: Zoning and Land Use Documentation
• Public Facilities, Non-Residential: Project Schedule
• Public Facilities Non-Residential: Project Schedule Narrative. In addition the project schedule, please provide a narrative description about how the timelines were determined for obtaining commitments for leveraged project financing, real estate closing, temporary and permanent relocation, construction commencement, construction completion, etc. Describe the plan for securing all financing and the plan to complete acquisition within 12 months.
• For Public Facilities, Non-Residential: Commitment letters for all leveraged funds/Letters of Support - Please include 2-3 Commitment letters or letters of support for your project from local beneficiaries, other interested citizens, and/or other partnering Agencies.
• IRS Form 990 (Return if Organization Exempt From Income Tax)
• Additional Information: Upload project management capacity documentation and anything else that is applicable to the project and/or will strengthen the application.
