For the Performance Plan section, please be aware that by indicating your quarterly goals you agree to reach these goals to the best of your ability. It is essential that you follow your plans to achieve your predicted goals. In the case that you deviate from your plans, it is at MOLGBTQA's discretion to determine the continuation or termination of your grant. 

Should you need to amend or modify your application, please keep the following FY24 MOLGBTQA contract terms in mind.

ARTICLE XIII – GRANT AMENDMENT(S)/MODIFICATION(S)

1. Under no circumstances shall the Grantee change any of the terms and conditions of the grant without written permission. The Grantee may submit a request for an amendment to the grant in cases where the amendment is material or is needed due to some unforeseen circumstance that prevents the Grantee from carrying out the originally agreed upon terms of the grant. To request an amendment, Grantee must contact Grant Administrator and Grant Monitor listed in ARTICLE XII- GRANT ADMINISTRATOR AND CONTACTS in writing via email, explaining the change requested and the reason for said change. All requests will be considered on a case-by-case basis and may require documentation of materiality and unforeseen circumstances. Grantees may not execute any actions or disburse any funds pursuant to amendment requests until such request is approved by the Grant Administrator. All grant amendments must be made prior to the close of business on June 30, 2024, the end of the third quarter.
2. All grant amendment(s) and modification(s) must be approved by the MOLGBTQA, Grant Administrator or their designee. Programmatic changes may also result in financial alterations to the grant agreement.
3. Please note that any changes to be made must first be approved by MOLGBTQA before implementation. Failure to obtain approval may lead to termination of the grant agreement.
