What is an EIN?

The Employer Identification Number (EIN), also known as the Federal Employer Identification Number (FEIN) or the Federal Tax Identification Number, is a unique nine-digit number assigned by the Internal Revenue Service (IRS) to business entities operating in the United States for the purposes of identification.

What is an UEI Number?

The unique entity identifier used in SAM.gov has changed.

On April 4, 2022, the unique entity identifier used across the federal government changed from the DUNS Number to the Unique Entity ID (generated by SAM.gov).

• The Unique Entity ID is a 12-character alphanumeric ID assigned to an entity by SAM.gov.
• As part of this transition, the DUNS Number has been removed from SAM.gov.
• Entity registration, searching, and data entry in SAM.gov now require use of the new Unique Entity ID.
• Existing registered entities can find their Unique Entity ID by following the steps here.
• New entities can get their Unique Entity ID at SAM.gov and, if required, complete an entity registration.

Registering in SAM.gov is free of charge. If you encounter any organizations or websites soliciting a fee or charge to acquire a UEI number or to charge you for registering in SAM.gov it may be a scam or fraudulent!

Please be sure to answer each question thoroughly. If a question is not applicable, please write "N/A" and a brief explanation, as the system will not allow you to submit your application if any question is left blank. Additional information is described in italics below each question. Additional instructions and resource materials can be found under the "Library" tab. If you submit your application, you may still make changes to your application up to the deadline. All changes made to your application after submittal, but before the application deadline, will automatically be saved - there is no need to resubmit.

Please submit the following requested documents as noted in the instructions below. Please note that documents marked as "Required" MUST have a document uploaded in that location in order for you to be able to submit your application. If a "Required" document does not apply to you or your organization, please upload a document explaining why that requirement does not apply:

• Board Resolution Non-profit agencies applying for CDBG funding must document endorsement of each proposal for funding by formal action of their Board of Directors, as appropriate. Copies of meeting minutes or a Board resolution granting permission to apply for CDBG funds and designating the appropriate individual to sign a contract with West Jordan. This formal action must occur prior to the application due date to be eligible.
• Department of Commerce Verification-Active Status Provide verification from the Department of Commerce showing Active Status for the Organization which is applying.
• W-9 Form (If a new CDBG applicant, OR if the agency moved or mailing address changed, please download, complete, and upload the template below.
• Section 504 Disability/Non-Discrimination Employment Information (If a new CDBG applicant, and available)
• Deed Restriction (If Applicable)
• Language Access Plan (If a new CDBG applicant, and available)
• Map of project Site and service area (if Applicable)

For Non-profits, please upload where applicable:

• Organizational Chart/List of Board members
• Articles of Incorporation/By Laws
• Audit or Financial Statement
• 501(c)(3) Tax Exemption letter
• Liability Insurance Certificates  (If a new CDBG applicant, and available. Subrecipients must provide proof of insurance including Automobile Insurance and Workman's comp) 

If Applicable:

• Construction Project Cost Estimate Please provide a detailed description of the budget for your project.
• Indirect Cost Plan/Certification Please provide your federally approved indirect cost plan if you have requested CDBG funds to pay for indirect costs in your budget.
