Required Document Uploads

• Proof of Identity -  (Passport, VI Driver's License, Military ID, Senior Citizen ID - Must upload one of the items listed.Include for both Applicant, Co-Applicant and all household members)
• Proof of Ownership - (Deed for home) If your name is not on the deed, please provide proof of made payments to the mortgagee on a regular basis over the past two years.
• Proof of Residency - (Utility Bill, Property Tax Bill, Mortgage Statement, Hook up/Work Order, Vehicle Registration and/or Vehicle Insurance - Must Upload two items listed. The applicant's physical address must be provided on the document. No PO Boxes.)
• Proof of Income and Hardship (1) - W2s for affected years (2020,2021, 2022 and/or 2023); Stamped tax Return for affected years (2020, 2021.2022, and/or 2023) Job Letter, Check stubs - Applicant, Co-Applicant and Household Members must provide proof of income. (Household members over the age of 18, who are not full time students must provide proof of income.)
