Entries may be submitted on behalf of the county, a private nonprofit corporation, or a county Subrecipient. The submission must have the signature of the Housing and/or Community Development Program Director or Manager. The entry can be financed through the Community Development Block Grant entitlement or state-administered small communities CDBG program, the HOME Program, a non-federal program, a local initiative, etc., but it must be located in a NACCED member county. HOME Awards Program entries must include HOME funds as part or all of the financing. Programs must be no more than three years old from the date of submission to be eligible.
All programs submitted for the NACCED Annual Awards of Excellence will be competitively evaluated by the NACCED Awards Committee using the points indicated for each of the following four criteria:

Tangible Results - The project or program should have proven measurable, tangible results that show attainment of projected goals and success in reaching the population to be served. Attention will be paid to the success of the project or program in leveraging resources as well as the degree to which the community was involved in design and implementation. (25 points maximum)
Innovation - The project or program should be unique and innovative for that county and/or should have been developed in response to a particular problem or need. Consideration will be given to the strength and originality of public/private partnerships. (25 points maximum)
Sustainability - The County should have plans to continue the program or project through staffing, funding, project revenue generation or other means of support. Points will be awarded based on long term viability of the project or program. (25 points maximum)
Transferability - The entry should have qualities that are adaptable for use in other counties. (25 points maximum)

The Planning/Policy/Program Management Award is meant to recognize actions that achieve better efficiencies within the operations of public agencies. It is geared to the way an agency is managed rather than a specific project or program. Examples include model performance measurement or monitoring systems or leading practices in affordable housing needs assessment. The Innovation Award is meant to recognize a project that may be small in nature but has achieved success in a particularly creative or cutting edge way. This award is geared to projects that are smaller in scale.
Please read each question carefully and be thorough yet concise in your answer. This is necessary not only for adequate evaluation but also for effective reference for other counties requesting information on winning programs. Bear in mind that the review team will appreciate conciseness, but that you need to make sure that enough information is provided to enable the reviewers to evaluate your submission on the basis of the criteria described above.
